Your child's participation in the activities of The Parish Trust.
I understand that, while involved, he/she will be under the control and care of the group leader and/or other adults approved by the charity (apart from stay and play groups such as Tommy's Tots) and that, while the staff in charge of the group will take all reasonable care of the children, they cannot necessarily be held responsible for any loss, damage or injury suffered by my child during, or as a result of the activity.

Medical Information

In the event of an illness or accident every effort will be made by the event leader or their assistant to contact you. If for whatever reason this is not possible, treatment will be given as instructed in accordance with the details you give on this form. Your child will receive dental, medical or surgical treatment, including anaesthetic or blood transfusion, as considered necessary by medical authorities present in the case of a serious incident. By completing this form, you are agreeing to this.

Photographic Images

During the course of our regular activities we may take photographs to be used both on the charity website and in future event publicity. In order to publish an image of your child we need parental permission. All such photographs will be handled in accordance with our Child Protection Guidelines and we will NOT publish any names or personal information.

I grant permission for The Parish Trust to publish images of my child in promotional publications, the website and charity notice boards.

If you do not give permission for your child’s photo to be published, please make sure that your child is aware of this.
